Assistance Animal Accommodation Request
Use this form to request approval to keep an assistance animal (emotional support or service animal) as a reasonable accommodation. An assistance animal is not a pet and is not charged a pet deposit or pet rent.
Documentation is required. Unless your disability and the need for the animal are obvious, we require reliable documentation from a licensed health professional who has a treating relationship with you, confirming (1) that you have a disability and (2) that the animal provides disability-related support. Instant online “ESA certificates” from websites with no treating relationship are not sufficient. You may upload documentation below, or provide it before approval.
